If the U.S. and North Korea went to war, what would happen and who would win?

Who would win would largely depend upon the price the US is willing to pay in blood.

North Korea has anticipated an invasion for more than 50 years, they have been digging tunnels ever since in which they can hide the entire population.

The US would be facing an enemy popping up from all over the place, even in areas they thought they had secured.

Aerial bombardments and artillery would be largely ineffective. It would be like the Battle of Iwo Jima only a thousand times worse.

Should the US be willing to pay a high price in casualties, they would surely win.

As for how the battle would unfold under various presidents, that doesn’t matter. The war plans have already been drawn up, their are no real options.

I had a wonderful and successful life in the U.S., wonderful children and grandchildren.
I have always taken the communist side of any debate, any discussion. It's my nature. This fact is in my file when I was interviewed for security clearance.
My first wife's family was a land-owning class of North Korea when the Japanese invaded Korea. Her family and her group moved to China to fight against the Japanese. Her father was a graduate of Whampao Military Academy in China and an officer of the Nationalist Chinese Army during WWII. She was born in Chungking, the wartime capital of China.
After WWII ended, the Korean faction of the Chinese Army went back to Korea. She was driven by military escort and guards everywhere they go. Synman Rhee had the opposition party leaders killed because he had U.S. support. Her father was killed and the body was never found.
My first wife was very anti-communist. On any discussion on communism, I always had to take the communist side as I am currently doing in soc.culture.china.

If this war is to happen it is not so easy for the American airpower to win.

Saying air bombardment is ineffective is also not really true as smart
bombs, guided missiles and satellite tracking and so on can still reach
them.

It will take more sorties and more times and more resistance to overcome.
There will be severe ground casualties but their resistance will be far
exceeded of their past war.

You are only talking the effects on North Korea, but North Korea will not limit their missiles/bombs to U.S. assets. South Korea, Japan will definitely be bombed and destroyed, and maybe even Taiwan, Guam, Okinawa,... The U.S. can not win. We have examples already in Vietnam War, wars in Afghanistan, Iraq, Yemen, Somalia, Sudan, Libya, all over the Middle East...

US, SK, Japan would kill hundreds of thousands or millions in NK and completely destroy their offensive abilities within a week afterward. Or maybe West attacks NK first, same outcome. US, SK, Japan would not even try to conquer NK territory, mainly because that would start war with China. China however would invade NK immediately after US, SK, Japan destroyed NK's offensive capabilities. West doesn't care what happens to NK territory. SK, they care, but not NK.

NK loses bigtime, West loses mostly from NK's first strike, China wins bigtime. Everyone knows this, so NK and West will not attack each other.
But that means NK should really consider China its opponent, not the West.
Only China would benefit from a conflict, and China's soldiers are lining NK's borders. NK should realize this. And so should China.

Missiles that can reach Tokyo can also reach Beijing and Shanghai. How is it in China's interest to have allowed NK to develop such missiles?

There'd be no war with North Korea. North Korea has nuclear weapons, and that should stop the U.S./South Korea/Japan from attacking North Korea.

If Iraq had nuclear weapons, the Middle East would be peaceful today.
